The wrong item does not get into the box by itself
A wrong shipment almost never comes from a single mistake. A pick list is printed, two similarly coded products sit side by side, the team is in a hurry, and the line is verified by eye. Each link in that chain looks reasonable on its own — but at the end of it the customer receives the wrong box, and the cost is paid three times over: freight, return, and trust.

Where it goes wrong, and what catches it
Each row below pairs a moment where dispatch goes wrong with the control that catches it.
